How to Check Eligibility for Ayushman Card?

  1. To Check your Eligibility for an Ayushman Card, visit the official website at https://pmjay.gov.in/.
  2. On the Main menu options Click on ‘Am I Eligible’ and proceed.
  3. Verify the captcha and enter the Mobile Number of the Applicant and verify with OTP.
  4. Now you have been redirected to a new page and there are two options here.
  5. Select Search by entering your Ration Card details in the first part and your mobile number and ration card number in the second part.
  6. Then your eligibility will be displayed on the screen.

How to Register for Mera PMJAY Ayushman Card?

  1. To Register as Self User Visit the Registration Page at https://setu.pmjay.gov.in/setu/registerUser.
  2. Fill out the Registration form by providing to Select or Entering all necessary details.
    • State.
    • District.
    • Mobile Number.
    • Email ID.
    • Name.
    • Gander.
    • Date of Birth.
  3. Proceed with the ‘Submit‘ option and verify with OTP.
  4. Now that you have successfully registered.

You will get confirmation of the details of the registration through SMS and Email as per provided by you.

  2. Enter your Mobile Number and Security Captcha Code on BIS Login Page.
  3. Click on ‘Validate‘ to login into your account.

How to Apply for Ayushman Card Online

  1. Open the official PMJAY website on a web browser https://pmjay.gov.in/.
  2. Click the button named “Am I Eligible.”.
  3. Enter your Aadhar Number to reach the main Application page.
  4. Fill out the application form with your Personal and Family Details, including your Name, Address, and any other information needed.
  5. Submit the application form now.
  6. The ABHA Card Number will be shown on the screen.
  7. Applications for members of your family will be validated in the coming days.
  8. On your Dashboard, look for the newly generated Ayushman Bharat Card.
